Job summary

Heart of Hospice, part of the LHC Group within the Optum family, is hiring an Afterhours Registered Nurse to deliver hospice care to patients and families after regular hours, nights and weekends. You'll perform triage, initial nursing evaluations within 48 hours, and afterhours visits while documenting care and coordinating with families. A valid RN license and a reliable vehicle are required.

Qualifications

• Current unrestricted RN licensure in the state of practice.
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.
• LA: a minimum of two years full-time RN experience; substitution with two years of hospice experience as LPN allowed.
• Preferred: CPR certification or ability to complete within 90 days of hire.
• Preferred: 1+ years of clinical experience.

Responsibilities

• Triage patient needs over the phone until an in-person nurse visit.
• Respond to crisis calls after hours with urgency.
• Make initial nursing evaluation within 48 hours of referral.
• Perform afterhours visits including admissions, death, recertification and routine visits.
• Identify patient/family needs and reassess at least every 15 days.
• Document problems, goals, interventions and responses to care.
• Function in any home setting irrespective of age, race, creed, color, sex, disability or financial condition.
