Job summary

Atrium Health's Wake Forest Baptist Medical Center – Nursing: Care Coordination is seeking a Part-time Case Manager.

The role involves identifying patients who would benefit from Case Management, assessing needs, developing and implementing a Plan of Care with the team, and coordinating discharge planning. RN licensure and BSN are required; MSN preferred; minimum 5 years of clinical experience with case management is preferred.

Qualifications

• Graduate from an accredited school of nursing.
• BSN required; MSN preferred.
• Minimum 5 years of relevant clinical experience; prior Case Management experience preferred.

Responsibilities

• Identify patients who would benefit from Case Management interventions.
• Assess data and interview patient/family to evaluate needs.
• Develop and implement Plan of Care with nursing to improve outcomes.
• Coordinate discharge planning with multidisciplinary team and providers.
• Monitor Plan of Care and adjust as needed.
• Document actions in EMR and communicate with payors.
