Qualifications

• MSN preferred.
• Minimum 5 years of relevant clinical experience; Case Management experience preferred.

Responsibilities

• Identifies patients who would benefit from Case Management interventions based on initial screening for discharge needs.
• Assesses data by interviewing patient/family and evaluating needs.
• Develops and implements a Plan of Care with nursing to achieve outcomes; coordinates discharge planning with the team.
• Monitors and updates Plan of Care; communicates with providers and payors.
• Participates in multi-disciplinary rounds to ensure patient progression and optimal care.
• Identifies system issues and suggests interventions to improve efficiency and reduce duplication.
• Monitors medical necessity and level of care for admissions and procedures.
• Engages in quality improvement activities to reduce variations in practice.
• Provides age-appropriate planning and communication per age guidelines.
