Advocate Health is seeking a part-time Registered Nurse Case Manager for the Wake Forest Baptist Medical Center - Nursing: Care Coordination department in North Carolina. The role requires a BSN, 5 years of clinical experience, and case management certification within 2 years.

Qualifications

• Graduate from accredited school of nursing.
• BSN required.
• MSN preferred.
• Minimum 5 years of relevant clinical experience.
• Current licensure to practice as RN in the state.
• Certification in case management recommended within 2 years.

Responsibilities

• Identifies patients who would benefit from Case Management interventions based on initial screening.
• Assesses data and interviews patient/family to evaluate needs.
• Ensures Plan of Care is developed with nursing and implemented to improve outcomes.
• Collaborates with patient, family, and multidisciplinary team to execute the Plan of Care.
• Coordinates discharge planning process and completes planning needs.
• Evaluates and adjusts the Plan of Care with the team as needed.
• Documents actions in EMR and communicates with providers and payors.
• Participates in multidisciplinary rounds and supports patient progression.
• Identifies system issues and proposes interventions to avoid duplications.
• Monitors medical necessity and level of care; notifies UR as needed.
• Provides age-appropriate planning and communication per guidelines.
