Job summary

Wake Forest Baptist Medical Center is hiring a Nursing: Care Coordination role. This full-time position involves 40 hours weekly with a Monday–Friday schedule and 40-hour weeks.

The role demands patient assessments, care plans, discharge planning, and collaboration with clinical teams to ensure high-quality, cost-effective care. The incumbent will lead data reporting on patient outcomes, coordinate complex transitions, and mentor frontline care management staff while adhering to regulatory

Qualifications

• 2 years of clinical nursing experience and 3 years of care management experience
• Registered Nurse license in the practice state
• ACMA, NACCM, or RN-BC certification within 2 years (preferred)
• Bachelor’s degree in nursing required

Responsibilities

• Perform comprehensive patient assessments and develop care plans
• Analyze and report patient outcomes and resource utilization
• Coordinate discharge planning for clinically complex patients
• Provide mentorship to frontline care management staff
• Advocate for patients and families within regulatory guidelines
• Coordinate referrals and transitions to ensure timely care progression
• Lead multidisciplinary rounds and collaborate with social work and other teams
• Engage with physician practices and payor stakeholders to optimize care
• Maintain knowledge of regulatory requirements and CMS notices
