Job summary

Advocate Health is seeking a part-time Case Manager RN for Wake Forest Baptist Medical Center—Nursing: Care Coordination. The role requires BSN, MSN preferred, and a minimum of 5 years of clinical experience with case management experience preferred.

Responsibilities include identifying patients for case management, assessing needs, developing/implementing a Plan of Care with nursing, and coordinating discharge planning. Strong communication and documentation in EMR are essential.

Qualifications

• Graduate from an accredited school of nursing; BSN required; MSN preferred.
• Minimum 5 years of relevant clinical experience.
• Case management experience preferred.

Responsibilities

• Identifies patients who would benefit from Case Management interventions based on discharge needs.
• Assesses data by interviewing patient/family and evaluating patient needs.
• Develops and implements a Plan of Care with nursing to achieve outcomes and coordinates with the team.
• Evaluates discharge planning effectiveness and updates Plan of Care as needed.
• Documents Case Management actions in the Electronic Medical Record and communicates with providers/payors.
• Participates in multi-disciplinary rounds to drive patient progression and coordinate with physicians.
• Identifies system issues and suggests interventions to avoid duplication and barriers.
• Monitors medical necessity and levels of care; notifies Utilization Review of changes when needed.
• Applies quality improvement principles to reduce practice variations.
• Provides age-appropriate patient planning and communication per guidelines.
