Job summary

Atrium Health Wake Forest Baptist is seeking a part-time Case Manager for the Lexington Medical Center – Nursing: Care Coordination. The role focuses on coordinating care across the patient care team to achieve optimal outcomes while advocating for patients and ensuring continuity of care.

The qualified candidate will be a registered nurse with BSN preferred and MSN encouraged, and should have at least five years in case management.

Qualifications

• Graduate of an accredited School of Registered Professional Nurses, BSN preferred, MSN encouraged.
• Minimum of five years of experience in the care of assigned patient population.
• Current registration to practice as a Registered Nurse in the applicable state.
• Case Management certification encouraged.

Responsibilities

• Utilizes the nursing process to develop, implement, and evaluate case management outcomes for defined patient populations.
• Collaborates with a patient care team including physicians, nurses, rehab, pharmacy, social work, and other disciplines to coordinate care.
• Analyzes patient care to identify opportunities for quality improvement and cost reduction based on current research.
