Job Summary

This position's primary focus is managing members with specialty (diabetes) diagnoses. The role involves performing all functions of case management (CM) and serving as a primary contact for members, healthcare personnel, and other entities involved in managing specialty care. The primary nurse case administrator performs care coordination, identifies alternative treatment programs, consults with physicians, providers, members, and resources to evaluate options and services needed to meet health needs, promotes quality and cost-effective outcomes, and acts as a liaison to physicians and members. The role also includes providing education and local resource information, encouraging member self-education, assessing members for case management, introducing members to website tools, educating about their specific conditions, and facilitating care coordination.

Job Requirements

• Registered Nurse (RN) with current, valid, unrestricted license in the state of operations
• At least 4 years of clinical practice experience, including 3 years of recent specialty (Diabetes) clinical experience
• Knowledge of specialty diagnosis drugs, adjunct therapies, treatment protocols, side effects, and complications
• Current certification in ONS/ONCC/AOCN/CPON/AOCNP/CDE/CDCDS
• Proficiency in Word, Excel, Lotus Notes, and database management
• Clear and concise verbal and written communication skills
• Knowledge of UM/CM/DM activities and standardized criteria
• Familiarity with ancillary services like HHC, SNF, Hospice, etc.
• Strong clinical judgment, analytical skills, and problem-solving abilities
• For positions requiring multi-state licenses, additional licenses must be obtained and maintained; fees are covered by HCSC
• Knowledge of drugs, treatment protocols, and dietary considerations
• Experience in setting SMART goals and healthy coping strategies
• Maintaining current certification for specialty specialists

Preferred Requirements

• Experience managing complex or catastrophic health cases
• Inpatient and outpatient experience
• 1-year experience in Care Management within health insurance or managed care
• Current CCM certification
• Knowledge of medical management policies and procedures
• 1-year education experience in the specialty area

This is a remote role: Must reside within 250 miles of the office or within the posted state.

Base Pay Range

$69,200.00 - $146,700.00
