This position's primary focus is managing members with specialty (diabetes) diagnoses. The role involves performing all functions of case management (CM) and serving as a primary contact for members, healthcare personnel, and other entities involved in managing specialty care. The primary nurse case administrator performs care coordination, identifies alternative treatment programs, consults with physicians, providers, members, and resources to evaluate options and services needed to meet health needs, promotes quality and cost-effective outcomes, and acts as a liaison to physicians and members. The role also includes providing education and local resource information, encouraging member self-education, assessing members for case management, introducing members to website tools, educating about their specific conditions, and facilitating care coordination.
This is a remote role: Must reside within 250 miles of the office or within the posted state.
