Job Summary

This position's primary focus is the management of members with specialty (diabetes) diagnoses. The role involves performing all functions of case management (CM) and serving as a primary contact for members, healthcare personnel, and other entities involved in managing specialty care. The primary nurse case administrator performs care coordination, identifies treatment options, consults with physicians, providers, members, and other resources to evaluate needs, promotes quality and cost-effective outcomes, and acts as a liaison to physicians and members. The role includes providing education and local resource information, encouraging member self-education, assessing members for case management, introducing members to website tools, educating about specific conditions, and facilitating care coordination.

Job Requirements
  • Registered Nurse (RN) with current, valid, unrestricted license in the state of operation
  • At least 4 years of clinical practice experience, including 3 years of recent specialty (diabetes) clinical experience
  • Knowledge of specialty diagnosis drugs, therapies, treatment protocols, side effects, and complications
  • Current certification in ONS/ONCC/AOCN/CPON/AOCNP/CDE/CDCDS
  • Proficiency in Word, Excel, Lotus Notes, and database management
  • Strong verbal and written communication skills
  • Knowledge of UM/CM/DM activities and standardized criteria
  • Familiarity with ancillary services such as HHC, SNF, Hospice, etc.
  • Analytical skills, clinical judgment, problem-solving, and ability to establish SMART goals
  • For positions requiring multi-state licenses, incumbents must obtain and maintain additional licenses as required by management
  • Knowledge of drugs, treatment protocols, and dietary considerations
  • Current Certified Specialists must maintain their certification
Preferred Job Requirements
  • Experience managing complex or catastrophic health cases
  • Inpatient and outpatient experience
  • 1 year of care management experience in a health insurance or managed care setting
  • Current CCM certification
  • Knowledge of medical management policies and procedures
  • 1 year of education experience in a specialty area

This is a Telecommute (Remote) role: Must reside within 250 miles of the office or anywhere within the posted state.

Base Pay Range

$69,200.00 - $146,700.00
