Job Summary

This position's primary focus is managing members with specialty (diabetes) diagnoses. The primary nurse case administrator performs all functions of case management (CM) and is a key contact for members, healthcare providers, and other involved entities. Responsibilities include care coordination, identifying treatment options, consulting with physicians and members, promoting quality and cost-effective outcomes, and serving as a liaison. The role also involves educating members, introducing website tools, and facilitating care coordination for identified members.

Job Requirements:

• Registered Nurse (RN) with current, valid, unrestricted license in the state of operation
• At least 4 years of clinical practice experience, including 3 years in recent Specialty (Diabetes) clinical care
• Knowledge of specialty diagnosis drugs, therapies, treatment protocols, side effects, and complications
• Current certification in ONS/ONCC/AOCN/CPON/AOCNP/CDE/CDCDS
• Proficiency in Word, Excel, Lotus Notes, and database management
• Excellent verbal and written communication skills
• Knowledge of Utilization Management/Case Management/ Disease Management activities and criteria
• Familiarity with ancillary services such as HHC, SNF, Hospice, etc.
• Strong clinical judgment, analytical skills, and problem-solving ability
• For multi-state licensed positions, additional licenses may be required; fees covered by HCSC
• Knowledge of diets related to member assistance
• Understanding of Problem Solving, Healthy Coping, and SMART goals
• Maintaining current certification if applicable

Preferred Qualifications:

• Experience managing complex or catastrophic health cases
• Inpatient and outpatient experience
• 1 year in Care Management within a health insurance/managed care setting
• Current CCM certification
• Knowledge of medical management policies and procedures
• 1 year of education experience in Specialty area

This is a remote role: Must reside within 250 miles of the office or within the posted state.

Base Pay Range

$69,200.00 - $146,700.00
