Job summary

Qualifications

• Bachelor's degree in Social Work or Human Service field or the equivalent in education and experience required.
• Excellent interpersonal, communication, and organizational skills required.
• BLS within 30 days.

Responsibilities

• Provides assistance to patients/families and health care providers to identify barriers to care and facilitate access to resources.
• Acts as an advocate in obtaining services and supports for eligible patients, especially when barriers affect health outcomes.
• Maintains and updates resource directories and educational materials for staff, patients, and families.
• Documents services, referrals, interventions, and patient interactions in medical records and databases.
• Collaborates with care teams and community agencies to address barriers to care and access to resources.
• Assists patients in navigating healthcare, community, and public assistance systems to improve health outcomes.
• Performs other duties as required.
