Qualifications

• Bachelors’ degree in psychology, social work or related field preferred.
• One year case management experience in a community, hospital or medical/behavioral health setting preferred.
• Experience with utilization screening tools preferred.

Responsibilities

• Collaborate with practice and health care teams.
• Outreaches individuals recently served by the SVMC Emergency Department.
• Conducts relevant screenings.
• Supports provider treatment plan and promotes adherence to physician/provider recommendations and instructions.
