Job summary

Dartmouth Health is seeking a dedicated social services professional to assist patients and families across the care continuum. You will advocate for access to needed services, maintain resource directories, and document outcomes in patient records within a hybrid remote framework.

Responsibilities include applying for programs like Medicaid, coordinating transportation, and guiding families through eligibility and financial assistance processes.

Qualifications

  • Bachelor's degree in Social Work or Human Service field or the equivalent in education and experience required.
  • Excellent interpersonal, communication, and organizational skills required.

  • Updates information resource information directory and maintains related literature & applications for distribution to staff and families in specific area of concentration.
  • Documents services provided in patient’s medical record and applicable databases.
  • Negotiates and advocates access for patients to selected skilled, acute or long term care facilities. Identifies available transportation resources and negotiates with patient/family for cost effective, safe transportation.
  • Works with patient/family to assess patient eligibility for medication, co-pay and additional resource assistance.
  • Facilitates the patients/families application to public and private government and community agencies, primarily Medicaid.
  • Provides information and assistance to patients/families to secure financial and health assistance, organization financial assistance, and other governmental programs.
  • Performs other duties as required or assigned.

  • BLS within 30 days

  • Pay Range: $46,321.60/Yr. - $71,780.80/Yr. (Based on 40 hours per week, otherwise pro rata)
