Job summary

Hometown Urgent Care in Columbus, OH is seeking a compassionate Primary Care Provider/Advanced Practice Provider (NP or PA) to deliver comprehensive primary care across the lifespan at our Easton location. You will perform exams, diagnose conditions, and collaborate with specialists to ensure continuity of care.

Ideal candidates are licensed NP-C/FNP or PA-C in Ohio, with outpatient experience and strong communication skills.

Qualifications

• Must be certified as NP-C/FNP or PA-C and licensed in Ohio.
• Frontline experience in family medicine or internal medicine preferred.
• Excellent communication and empathy with diverse patients.

Responsibilities

• Conduct comprehensive physical exams, assessments, and patient histories.
• Diagnose and manage a wide range of conditions.
• Provide primary care support across multiple specialties.
• Perform in-scope procedures.
• Collaborate with ENT, Surgical, and other subspecialties for coordinated care.
• Utilize E-ClinicalWorks for documentation.
• Provide emergency and urgent interventions when needed.
• Educate patients and families on treatment plans and prevention.

We are seeking acompassionate and skilled Primary Care Provider/Advanced Practice Provider (NP or PA)to provide comprehensive primary care services to patients across the lifespan. What You'll Do:

• Conduct comprehensive physical exams, assessments, and patient histories
• Diagnose and manage a wide range of conditions
• Provide primary care support across multiple specialties
• Perform in-scope procedures
• Collaborate closely with specialists inENT, Surgical, and Medical Subspecialtiesto ensure continuity and coordination of care
• UtilizeE-ClinicalWorksfor efficient, compliant, and patient-centered documentation
• Provide emergency and urgent interventions when necessary
• Educate patients and families on treatment plans, prevention strategies, and lifestyle modifications

• CertifiedNurse Practitioner (NP-C/FNP) or Physician Assistant (PA-C)licensed in the state of Ohio
• Must be experienced in primary care, family medicine, or internal medicine
• Comfortable managing a broad patient panel across all ages and backgrounds
• Proficient in clinical procedures relevant to outpatient care
• Strong communication and empathy skills—your ability toconnectwith patients matters as much as your clinical expertise
