Job summary

Hometown Urgent Care & Occupational Health is seeking a compassionate Primary Care Provider/Advanced Practice Provider (NP or PA) for our Easton location to deliver comprehensive primary care across the lifespan. You will diagnose, manage, and educate patients, using a patient-centered, team-based approach.

The role emphasizes prevention, treatment, and collaboration with specialists to ensure coordinated care. Proficiency with E-ClinicalWorks is preferred, and Ohio licensure is required.

Qualifications

• Licensed NP-C/FNP or PA-C in Ohio.
• Experience in primary care, family medicine, or internal medicine.
• Comfort managing a broad patient panel across all ages.

Responsibilities

• Conduct comprehensive physical exams, assessments and patient histories.
• Diagnose and manage conditions across a wide range.
• Provide primary care across multiple specialties and perform in-scope procedures.
• Collaborate with ENT, Surgical and Medical Subspecialties for continuity of care.
• Educate patients on treatment plans, prevention and lifestyle changes.
• Utilize E-ClinicalWorks for documentation.
• Provide emergency and urgent interventions when necessary.

Job description

We are seeking a compassionate and skilled Primary Care Provider/Advanced Practice Provider (NP or PA) to provide comprehensive primary care services to patients across the lifespan. What You'll Do:

• Conduct comprehensive physical exams, assessments, and patient histories
• Diagnose and manage a wide range of conditions
• Provide primary care support across multiple specialties
• Perform in-scope procedures
• Collaborate closely with specialists in ENT, Surgical, and Medical Subspecialties to ensure continuity and coordination of care
• Utilize E-ClinicalWorks for efficient, compliant, and patient-centered documentation
• Provide emergency and urgent interventions when necessary
• Educate patients and families on treatment plans, prevention strategies, and lifestyle modifications

Requirements

• Certified Nurse Practitioner (NP-C/FNP) or Physician Assistant (PA-C) licensed in the state of Ohio
• Must be experienced in primary care, family medicine, or internal medicine
• Comfortable managing a broad patient panel across all ages and backgrounds
• Proficient in clinical procedures relevant to outpatient care
• Strong communication and empathy skills—your ability to connect with patients matters as much as your clinical expertise
