Job summary
Optimal Care is seeking a Physician Services clinician to deliver advanced primary and specialty care to geriatric and medically complex patients, often in their homes or care settings, with autonomy to emphasize thoughtful, patient-centered care that improves comfort and quality of life.
You will perform comprehensive evaluations, diagnose and manage conditions, develop individualized treatment plans, coordinate with patients, families and interdisciplinary teams, and provide medical leadership
Qualifications
- Doctor of Medicine or Osteopathy required.
- Current unrestricted State license to practice medicine.
- Board certification in family practice, internal medicine, geriatric or palliative care.
- Reliable transportation, valid driver’s license, and automobile insurance.
Responsibilities
- Conduct comprehensive medical evaluations.
- Diagnose and manage acute and chronic conditions.
- Develop individualized treatment plans and coordinate care with patients, families, and interdisciplinary teams.
- Provide medical leadership and mentoring for Nurse Practitioners and Physician Assistants.
- Help improve patient outcomes and reduce hospitalizations.
