Job summary

Optimal Care - Evergreen seeks a Physician in Physician Services to deliver advanced primary and specialty care to geriatric and medically complex patients, often in homes or care settings, with autonomy to focus on thoughtful, patient-centered care that improves comfort and quality of life.

You will supervise NPs and PAs, lead clinical teams, and collaborate with families and other clinicians to optimize patient outcomes and care coordination.

• Understands regulations and reimbursement in home health care and provides leadership.

Responsibilities

• Provide advanced primary and specialty care to geriatric and medically complex patients, often in homes or care settings.
• Conduct comprehensive medical evaluations, diagnose and manage conditions, and develop individualized treatment plans.
• Coordinate care with patients, families, and interdisciplinary teams; mentor NPs and PAs.
• Lead clinical quality initiatives to improve outcomes and reduce hospitalizations.
