Job summary

Optimal Care seeks a physician to deliver advanced primary and specialty care, often in patients’ homes or care settings. You will diagnose, manage conditions, and lead a care team to ensure thoughtful, patient-centered outcomes.

You will mentor Nurse Practitioners and Physician Assistants, coordinate with families, and help reduce hospitalizations while improving quality of life for patients and their families. Extensive travel within service areas is required.

Qualifications

• Must hold an unrestricted State medical license.
• Board certification in family medicine, internal medicine, geriatric, or palliative care.
• Prior supervisor/management experience required.
• Reliable transportation and valid driver's license.

Responsibilities

• Provide advanced primary and specialty care to geriatric and medically complex patients.
• Conduct comprehensive medical evaluations and manage acute/chronic conditions.
• Develop individualized treatment plans and coordinate care with patients and families.
• Provide medical leadership and oversight for NPs and PAs; mentor clinicians.
• Improve patient outcomes and reduce hospitalizations through coordinated care.
