Job summary

Optimal Care is seeking a Physician in Physician Services to deliver advanced primary and specialty care to geriatric and medically complex patients, often in their homes or care settings, with autonomy to focus on thoughtful, patient-centered care.

You will conduct comprehensive medical evaluations, diagnose and manage acute and chronic conditions, develop individualized treatment plans, and coordinate care with patients, families, and interdisciplinary healthcare teams.

Qualifications

• Doctor of Medicine or Osteopathy is required.
• Current unrestricted State license to practice medicine.
• Board certification in family practice, internal medicine, geriatric or palliative care.
• Prior supervisor/management experience.
• Reliable transportation, valid driver’s license, and valid automobile insurance.
