Upperline Health in Central Florida seeks a Primary Care Physician (MD/DO) for its mobile physician group delivering care at Assisted Living Facilities. Practice thoughtful, value-based medicine within an integrated team focused on chronic disease management.

This full-time role features Monday–Friday, 8am–5pm, no nights or weekends, shared provider call, and opportunities for leadership and career growth as the organization expands its innovative care model.

Qualifications

• MD or DO required.
• Accredited Internal Medicine or Family Medicine residency completed.
• Board certification or board eligibility required.
• Active medical license preferred or ability to obtain in timeframe.
• DEA registration or eligibility required.
• BLS certification and maintenance.
• Open to experienced or new graduates.

Responsibilities

• Provide comprehensive primary care to a defined patient population, many with chronic conditions such as diabetes, vascular disease and mobility limitations.
• Diagnose conditions, develop treatment plans, and prescribe medications and therapies in coordination with Upperline Health specialists.
• Conduct patient assessments through history, exams, and diagnostic evaluation.
• Order, review, and interpret laboratory tests and imaging.
• Coordinate and manage referrals to specialists and follow up on patient outcomes.
• Educate patients and families on diagnoses, treatment options, medications, and preventive health strategies.
• Maintain timely, accurate clinical documentation in the EHR.
• Comply with clinical policies, quality standards, and regulatory requirements.

We are seeking a Primary Care Physician (MD/DO) for our mobile physician group who wants to practice thoughtful, value-based care medicine. This group visits Assisted Living Facilities throughout Central Florida.

This is an opportunity to provide professional physician services in an integrated specialty and primary care organization, focused on chronic disease management, prevention, and long‑term patient outcomes.

Upperline Health providers coordinate patients’ care among a team of specialists - Podiatric Physicians and Surgeons, Primary Care Physicians, Endocrinologists, Advanced Practice Providers, Care Navigators, Pharmacists, Dieticians, and Social Workers for integrated treatment that addresses patients’ immediate and long‑term health needs. What you’ll do:

• Provide comprehensive primary care to a defined patient population, many with chronic conditions such as diabetes, vascular disease and mobility limitations.
• Diagnose conditions, develop treatment plans, and prescribe medications and therapies as appropriate, working with Upperline Health specialists and care teams to proactively manage conditions, reduce complications, and improve quality of life.
• Conduct patient assessments through medical history, physical exams, and diagnostic evaluation.
• Order, review, and interpret laboratory tests and diagnostic imaging.
• Coordinate and manage referrals to specialists and follow up on patient outcomes.
• Educate patients and families on diagnoses, treatment options, medications, and preventive health strategies.
• Maintain timely, accurate clinical documentation in the electronic health record.
• Comply with clinical policies, quality standards, and regulatory requirements.

Experience and Qualifications

• Medical Doctor or Doctor of Osteopathic Medicine required
• Successful completion of accredited Internal Medicine or Family Medicine residency program required
• Board certification or board eligibility required
• Medical license required - active license preferred or the ability to obtain within applicable timeframe
• Possession of DEA registration or eligibility required
• BLS certification and maintenance
• Open to experienced or new graduate providers
