Job summary

Upperline Health is seeking a Primary Care Physician (MD/DO) for our Starke, FL clinic. This outpatient, clinic-based role emphasizes thoughtful, value-based care for medically complex patients with chronic disease management and long-term outcomes.

Expect predictable hours (Mon-Fri 8am-5pm), no nights or weekends, a manageable patient panel, and collaboration within a multidisciplinary team to coordinate care across specialties for proactive, patient-centered treatment.

Qualifications

  • Medical Doctor or Doctor of Osteopathic Medicine required
  • Successful completion of accredited Internal Medicine or Family Medicine residency program required
  • Board certification or board eligibility required
  • Medical license required - active license preferred or the ability to obtain within applicable timeframe
  • Possession of DEA registration or eligibility required
  • BLS certification and maintenance
  • Open to experienced or new graduate providers

Responsibilities

  • Provide comprehensive primary care to a defined patient population, many with chronic conditions such as diabetes and vascular disease.
  • Diagnose conditions, develop treatment plans, and prescribe medications and therapies as appropriate, working with Upperline Health specialists and care teams to proactively manage conditions.
  • Conduct patient assessments through medical history, physical exams, and diagnostic evaluation.
  • Order, review, and interpret laboratory tests and diagnostic imaging.
  • Coordinate and manage referrals to specialists and follow up on patient outcomes.
  • Educate patients and families on diagnoses, treatment options, medications, and preventive health strategies.
  • Maintain timely, accurate clinical documentation in the electronic health record.
  • Comply with clinical policies, quality standards, and regulatory requirements.

This position provides a unique outpatient‑only opportunity to practice within a multidisciplinary care model designed to proactively manage primary care needs along with Chronic Disease Management. Upperline Health providers coordinate patients’ care among a team of specialists - Podiatric Physicians and Surgeons, Primary Care Physicians, Endocrinologists, Advanced Practice Providers, Care Navigators, Pharmacists, Dieticians, and Social Workers for integrated treatment that addresses patients’ immediate and long‑term health needs. We put patients at the center of value‑based care. This is a unique role designed for physicians who want more than traditional primary care.

What you’ll do:
