Job summary

Upperline Health is seeking a Primary Care Physician (MD/DO) to join our mobile physician group in Central Florida, visiting Assisted Living Facilities and delivering value-based care. Monday–Friday, 8am–5pm, with no nights or weekends.

We offer a $50K sign-on bonus, market-leading pay up to $300K annually, and a comprehensive benefits package including health, dental, vision, 401K matching, PTO and parental leave. Licensure, malpractice, and CME support are included.

Qualifications

• MD/DO required and board-certified or eligible.
• Completed accredited Internal Medicine or Family Medicine residency.
• Active medical license or ability to obtain within timeframe.
• DEA registration or eligibility.
• BLS certification and maintenance.
• Open to experienced or new graduates.

Responsibilities

• Provide comprehensive primary care to a defined patient population, many with chronic conditions.
• Diagnose, treat, and prescribe medications; coordinate with Upperline Health specialists to manage conditions.
• Conduct patient history, physicals, and diagnostic assessments.
• Order, review, and interpret labs and imaging.
• Coordinate referrals and follow up on patient outcomes.
• Educate patients on diagnoses, treatments, and prevention strategies.
• Maintain accurate EHR documentation and comply with policies.

We are seeking aPrimary Care Physician (MD/DO)for our mobile physician group who wants to practice thoughtful, value‑based care medicine. This group visits Assisted Living Facilities throughout Central Florida.

What you’ll do:

• Provide comprehensive primary care to a defined patient population, many with chronic conditions such as diabetes, vascular disease and mobility limitations.
• Diagnose conditions, develop treatment plans, and prescribe medications and therapies as appropriate, working with Upperline Health specialists and care teams to proactively manage conditions, reduce complications, and improve quality of life.
• Conduct patient assessments through medical history, physical exams, and diagnostic evaluation.
• Order, review, and interpret laboratory tests and diagnostic imaging.
• Coordinate and manage referrals to specialists and follow up on patient outcomes.
• Educate patients and families on diagnoses, treatment options, medications, and preventive health strategies.
• Maintain timely, accurate clinical documentation in the electronic health record.
• Comply with clinical policies, quality standards, and regulatory requirements.

Experience and Qualifications

• Medical Doctor or Doctor of Osteopathic Medicine required
• Successful completion of accredited Internal Medicine or Family Medicine residency program required
• Board certification or board eligibility required
• Medical license required – active license preferred or the ability to obtain within applicable timeframe
• Possession of DEA registration or eligibility required
• BLS certification and maintenance
• Open to experienced or new graduate providers
