Job summary

Upperline Health in Daytona Beach, FL is seeking a Primary Care Physician (MD/DO) to practice value-based, outcomes-driven care in a clinic setting.

You will provide comprehensive primary care to patients with chronic disease management, coordinated with a multidisciplinary team, with hours 8am-5pm Monday to Friday, no nights or weekends. This outpatient role offers career growth, leadership development, and a collaborative environment.

Qualifications

• MD or DO required and board-certified or eligible.
• Active license preferred or ability to obtain in timeframe.
• DEA registration or eligibility required.
• BLS certification maintained.
• Accredited Internal Medicine or Family Medicine residency completed.

Responsibilities

• Provide comprehensive primary care to patients with chronic conditions.
• Diagnose, develop treatment plans, and coordinate with care teams.
• Conduct patient assessments and order necessary tests.
• Review and interpret lab results and imaging.
• Coordinate referrals and follow-up on outcomes.
• Educate patients on diagnoses, options, medications, and prevention.
• Maintain accurate EHR documentation and comply with policies.

This is a clinic-based role within an integrated specialty and primary care organization, focused on chronic disease management, prevention, and long-term patient outcomes.

This position provides a unique outpatient-only opportunity to practice within a multidisciplinary care model designed to proactively manage primary care needs along with Chronic Disease Management. Upperline Health providers coordinate patients’ care among a team of specialists – Podiatric Physicians and Surgeons, Primary Care Physicians, Endocrinologists, Advanced Practice Providers, Care Navigators, Pharmacists, Dieticians, and Social Workers for integrated treatment that addresses patients’ immediate and long-term health needs. What you’ll do:

• Provide comprehensive primary care to a defined patient population, many with chronic conditions such as diabetes, vascular disease and mobility limitations.
• Diagnose conditions, develop treatment plans, and prescribe medications and therapies as appropriate, working with Upperline Health specialists and care teams to proactively manage conditions, reduce complications, and improve quality of life.
• Conduct patient assessments through medical history, physical exams, and diagnostic evaluation.
• Order, review, and interpret laboratory tests and diagnostic imaging.
• Coordinate and manage referrals to specialists and follow up on patient outcomes.
• Educate patients and families on diagnoses, treatment options, medications, and preventive health strategies.
• Maintain timely, accurate clinical documentation in the electronic health record.
• Comply with clinical policies, quality standards, and regulatory requirements.

Experience and Qualifications

• Medical Doctor or Doctor of Osteopathic Medicine required
• Successful completion of accredited Internal Medicine or Family Medicine residency program required
• Board certification or board eligibility required
• Medical license required – active license preferred or the ability to obtain within applicable timeframe
• Possession of DEA registration or eligibility required
• BLS certification and maintenance
• Open to experienced or new graduate providers
