Upperline Health is seeking a Primary Care Physician (MD/DO) for the Daytona Beach, FL clinic. This clinic-based role sits within an integrated team focused on chronic disease management and prevention, with a value-based approach.

Work hours are Monday–Friday, 8am–5pm, with no nights or weekends, offering predictable workload and career growth within a multidisciplinary care model. Competitive compensation up to $275K, plus a $50K signing bonus and comprehensive benefits including

• MD/DO degree with Internal Medicine or Family Medicine residency.
• Board certification or eligibility required.
• Active medical license or ability to obtain within applicable timeframe.
• DEA registration or eligibility.
• BLS certification required.
• Open to experienced or new graduate providers.

Responsibilities

• Provide comprehensive primary care to a defined patient population.
• Diagnose conditions, develop treatment plans, and prescribe medications as appropriate.
• Coordinate and manage referrals to specialists and follow up on outcomes.
• Educate patients on diagnoses, treatments and prevention strategies.

This is a clinic-based role within an integrated specialty and primary care organization, focused on chronic disease management, prevention, and long‑term patient outcomes.

This position provides a unique outpatient‑only opportunity to practice within a multidisciplinary care model designed to proactively manage primary care needs along with Chronic Disease Management. Upperline Health providers coordinate patients’ care among a team of specialists – Podiatric Physicians and Surgeons, Primary Care Physicians, Endocrinologists, Advanced Practice Providers, Care Navigators, Pharmacists, Dieticians, and Social Workers for integrated treatment that addresses patients’ immediate and long‑term health needs. We put patients at the center of value‑based care. This is a unique role designed for physicians who want more than traditional primary care.

What you’ll do:

• Provide comprehensive primary care to a defined patient population, many with chronic conditions such as diabetes, vascular disease and mobility limitations.
• Diagnose conditions, develop treatment plans, and prescribe medications and therapies as appropriate, working with Upperline Health specialists and care teams to proactively manage conditions, reduce complications, and improve quality of life.
• Conduct patient assessments through medical history, physical exams, and diagnostic evaluation.
• Order, review, and interpret laboratory tests and diagnostic imaging.
• Coordinate and manage referrals to specialists and follow up on patient outcomes.
• Educate patients and families on diagnoses, treatment options, medications, and preventive health strategies.
• Maintain timely, accurate clinical documentation in the electronic health record.
• Comply with clinical policies, quality standards, and regulatory requirements.

Experience and Qualifications

• Medical Doctor or Doctor of Osteopathic Medicine required
• Successful completion of accredited Internal Medicine or Family Medicine residency program required
• Board certification or board eligibility required
• Medical license required – active license preferred or the ability to obtain within applicable timeframe
• Possession of DEA registration or eligibility required
• BLS certification and maintenance
• Open to experienced or new graduate providers
