Job summary

Upperline Health is seeking a Primary Care Physician (MD/DO) for its Daytona Beach, FL clinic to deliver thoughtful, value-based care within a multidisciplinary team. This outpatient role emphasizes chronic disease management, prevention, and long-term outcomes.

The position offers predictable hours (Mon-Fri, 8am–5pm), no nights or weekends, and opportunity for leadership growth within a growing organization. Competitive compensation and a $50K sign-on are provided.

Qualifications

• MD or DO degree required.
• Residency in Internal Medicine or Family Medicine completed.
• Board certification or eligibility required.
• Active medical license preferred or obtainable within timeframe.
• DEA registration or eligibility required.
• BLS certification and maintenance required.
• Open to both experienced and new graduates.

Responsibilities

• Provide comprehensive primary care to a defined patient population, including those with chronic conditions.
• Diagnose conditions, develop treatment plans, and prescribe therapies in coordination with care teams.
• Conduct patient assessments via history, exams, and diagnostics.
• Order, review, and interpret labs and imaging.
• Coordinate referrals and follow-up on outcomes.
• Educate patients and families on diagnoses and preventive care.
• Maintain accurate EHR documentation.
• Comply with policies, quality standards, and regulatory requirements.

This position provides a unique outpatient-only opportunity to practice within a multidisciplinary care model designed to proactively manage primary care needs along with Chronic Disease Management. Upperline Health providers coordinate patients’ care among a team of specialists - Podiatric Physicians and Surgeons, Primary Care Physicians, Endocrinologists, Advanced Practice Providers, Care Navigators, Pharmacists, Dieticians, and Social Workers for integrated treatment that addresses patients’ immediate and long-term health needs. What you’ll do:

• Provide comprehensive primary care to a defined patient population, many with chronic conditions such as diabetes, vascular disease and mobility limitations.
• Diagnose conditions, develop treatment plans, and prescribe medications and therapies as appropriate, working with Upperline Health specialists and care teams to proactively manage conditions, reduce complications, and improve quality of life.
• Conduct patient assessments through medical history, physical exams, and diagnostic evaluation.
• Order, review, and interpret laboratory tests and diagnostic imaging.
• Coordinate and manage referrals to specialists and follow up on patient outcomes.
• Educate patients and families on diagnoses, treatment options, medications, and preventive health strategies.
• Maintain timely, accurate clinical documentation in the electronic health record.
• Comply with clinical policies, quality standards, and regulatory requirements.

Experience and Qualifications

• Medical Doctor or Doctor of Osteopathic Medicine required
• Successful completion of accredited Internal Medicine or Family Medicine residency program required
