Our innovative care model is designed to foster meaningful engagements with patients, offering more time per visit and a manageable daily census. Physicians are supported by an interdisciplinary team of Nurse Practitioners (NPs), Physician Assistants (PAs), Medical Assistants, Social Workers, and dedicated referral and pharmacy support. With a focus on value-based care (VBC), this role allows you to practice medicine without the pressure of RVU targets, enabling you to deliver the highest standard of care.

Primary Care Physician - Summary

• Outpatient primary care for adults aged 65 and older

• Conduct extended visits (20–40 minutes each) to enable thorough assessments and patient education
• Work collaboratively with NPs, PAs, Medical Assistants, Social Workers, and pharmacy staff
• Focus on preventive medicine, chronic disease management, and care coordination
• Utilize EMR systems efficiently for documentation and workflow optimization
• Participate in continuous training and quality improvement initiatives
• Engage in regular team meetings and case reviews to ensure best practices
• Support integrated care models and contribute to an impact-driven organizational culture
