Are you seeking a fulfilling career as a Primary Care Physician focused on adult patients, specifically those aged 65 and older? This is a dynamic opportunity to join a progressive, value-based care organization dedicated to delivering high-quality preventive primary care. As a Primary Care Physician, you will be empowered to develop long-term, meaningful relationships with your patients while practicing in an environment that emphasizes quality over volume. This outpatient role ensures you have the support and resources necessary to make a significant impact on senior health outcomes, without the burden of RVU-based pressures.
In this Primary Care Physician position, you will work with an integrated care team that includes nurse practitioners, physician assistants, medical assistants, social workers, and pharmacy support, all collaborating to provide comprehensive care for adults 65 and older. The patient population primarily consists of Medicare Advantage participants, focusing on chronic disease management and preventive health services. With a manageable daily patient census of 12–15 and extended appointment times ranging from 20 to 40 minutes, you will have the time and support to deliver truly personalized care.
Joining this organization means you will benefit from a robust onboarding and professional development program, including a comprehensive three-week Physician Learning Journey. This onboarding covers all aspects of value-based care, documentation best practices, EMR optimization (Athena platform in transition), and integrated care model workflows. Whether you are experienced in value-based care or eager to learn, you will receive the training and mentorship necessary to excel in this innovative care delivery model.
The Primary Care Physician role is ideal for board certified or board eligible individuals in Internal Medicine or Family Medicine. While experience in geriatrics and HEDIS measures is preferred, it is not required; training will be provided. A passion for improving health outcomes for older adults and a collaborative, team-oriented mindset are key qualities that will ensure your success in this role.
Professionals in this position enjoy a comprehensive compensation package, including a competitive base salary, performance bonuses, commencement and sign-on bonuses, occurrence-based malpractice insurance, and full benefits from day one. Relocation assistance is available for qualifying moves, and the supportive, impact-driven work environment enables you to focus on delivering the best possible care for your patients.
