Primary Care Physician (PCP) - Summary

Duties & Responsibilities

• Deliver preventive and comprehensive primary care to a patient panel of adults 65+ (primarily Medicare Advantage)
• Conduct detailed patient assessments, develop individualized care plans, and manage chronic conditions
• Average daily census of 12-15 patients, with 20-40 minutes scheduled per visit for patient‑centered care
• Collaborate with NPs, PAs, medical assistants, social workers, and pharmacy/lab staff within an integrated care model
• Provide leadership within the care team, supporting best practices and quality initiatives
• Participate in performance improvement initiatives and adhere to documentation standards
• Engage in professional training, including a comprehensive 3‑week onboarding journey covering value‑based care, EMR use, and integrated workflows
