Job summary

Oak Street Health is seeking a Medical Scribe to support our primary care providers by documenting patient encounters in real time and supporting clinical documentation. This full-time role emphasizes accurate coding (CPT/ICD-10) and collaboration with the care team in a value-based care setting.

Ideal candidates commit 1–2 years to the role, have strong listening and communication skills, and are proficient with EHR workflows. In-person clinic work is expected in Cincinnati area.

Qualifications

• Knowledge of medical terminology and common medications.
• Prior clinical experience, including shadowing/volunteering, preferred.
• Scribe or transcription experience preferred but not required.

Responsibilities

• Document patient encounters in a structured note with history, assessment, plan, and exam.
• Assign CPT and ICD-10 codes.
• Prepare After Visit Summaries and support daily huddles/reviews.
• Provide administrative support to the care team and clinicians.
