Job summary

Equitas Health seeks a Prevention Health Navigator to implement health navigation services across onsite and offsite settings. You will recruit, enroll, screen, assess, and coordinate referrals to ensure access to medical services and health insurance for clients at high risk of HIV or living with HIV/AIDS.

Responsibilities include client-centered HIV test counseling, community education, and collaboration with local organizations.

Qualifications

• High School Diploma or equivalent preferred.
• Expertise with LGBTQIA+ communities required.
• Medical Assistant Certificate is an asset.
• Strong customer service skills and/or experience preferred.
• Experience related to community engagement, advocacy, or organizing preferred.
• Strong understating and commitment to anti-oppression principles and values.
• Must have a reliable means of transportation and adequate insurance, as statewide travel will be required.

Responsibilities

• Provide high quality health navigation for clients by helping access medical services and insurance.
• Conduct HIV prevention interventions per grant guidelines.
• Present to local/state HIV prevention groups.
• Recruitment via collaboration with community resources and partner sites.
• Plan and evaluate program effectiveness and fidelity.
• Provide HIV test counseling and linkage to care.
• Build relationships with funders and community organizations.
