Job summary

Via Care Community Health Center seeks a PrEP/PEP Navigator to educate, test, and coordinate HIV prevention services for at-risk individuals. You will help clients access PrEP/PEP, arrange labs, secure financial assistance, and support adherence.

Working with clinical staff and community partners, you will conduct rapid HIV testing on-site and in the community, document activities, and report outcomes to the Department Director.

Qualifications

• High School Diploma or GED required.
• At least 1 year of experience as Patient Navigator, Case Management, Education, or Human Services.
• Active Basic 1 Rapid HIV Testing certification from LA County DHSP required.
• Bilingual Spanish encouraged.
• Knowledge of PrEP/PEP and HIPAA is necessary.

Responsibilities

• Provide confidential risk assessments to determine HIV prevention needs.
• Provide access to PrEP/PEP and ensure adherence and follow-up.
• Screen for STIs and facilitate testing.
• Conduct rapid HIV testing on-site and in community settings.
• Report outcomes and maintain records; data entry into Casewatch.
• Engage in community outreach and health education.
• Assist in documenting program activities and coordinating care.
• Provide first-time medication drop-off within 5–10 miles of client.
