Ryan Health seeks an Experienced PrEP Navigator to promote PrEP/PEP access and support HIV prevention in New York City. The role focuses on identifying high‑risk individuals and linking them to care.

You will educate, screen for PrEP eligibility, provide navigation between clinics and community resources, and ensure follow‑up and appointment adherence.

A 4‑year degree and strong communication skills are required; bilingual abilities (Spanish or French) are a plus for serving diverse communities.

Competenze

• BSW or BA in Health or Human Services or equivalent experience required.
• Testing experience required.
• At least two years of experience providing educational or social services to HIV patients.
• Effective communication and writing skills.
• Proven track record of organization and attention to detail, response to requests.
• Proficient in Microsoft Word, Excel, Outlook, and PowerPoint.

• Promote PrEP services in venues with focus on YMSM of color.
• Educate community members about PrEP and PEP services.
• Recruit patients for PrEP services among HRNs.
• Complete non-medical assessments for PrEP services.
• Develop linkages to primary care, mental health, drug treatment programs and social services.
• Provide navigation to PrEP/PEP and related services.
• Conduct care planning and follow-up for PrEP patients.
• Advocate for enrollment in insurance assistance programs.
• Implement quality and performance improvement for PrEP services.
• Conduct HIV Testing using available methods.
• Provide crisis intervention and referrals to emergency services.
• Refer patients with psychiatric problems to specialists.
• Collaborate with team to identify service gaps and plan care.
• Provide home, hospital or community visits as appropriate.
