Qualifications

• Planning and documentation of care plans, teaching, and transition needs.

• Ongoing evaluation and reassessment of patient needs and responses.
• Proficient bedside shift reporting and handoffs to ensure continuity of care.
• Maintains safe patient environment and reports hazards.

Responsibilities

• Assess patients on admission and ongoing, providing comprehensive nursing care.
• Design, direct, evaluate, and document care plans and patient/family teaching.
• Prepare patients for treatments and report significant changes to team/physician.
• Reassess patients after interventions and adjust care as needed.
• Provide accurate shift reports to ensure continuity of care.
• Maintain safe and clean environment and adhere to safety protocols.
