Job summary

Chase Brexton Health Care seeks an Outreach and Care Coordination Specialist to support pregnant patients and newborns in the first year of life. The role helps patients access prenatal care, assists with insurance enrollment, and coordinates wrap-around services.

Based at Chase Brexton Glen Burnie with travel to Columbia centers and community events, the position requires strong communication, documentation, and HIPAA awareness; bilingual English/Spanish is a plus.

Qualifications

• Some college or equivalent work experience.
• Preferred: degree in health sciences or related field.
• One year experience working in the medical field.
• Experience working with a culturally and economically diverse population.
• Bilingual English and Spanish is a plus.

Responsibilities

• Act as a liaison between patients, providers, hospital care managers, external programs and resources.
• Assist patients with insurance enrollment and care coordination.
• Provide education, support and outreach to retain prenatal care and engage newborns in pediatric care.
• Document medical data accurately in the EMR and coordinate within care teams.
• Travel to community events and Chase Brexton locations as needed.
