Qualifications

Responsibilities

• Act as a liaison between patients, providers, hospital care managers, external programs and resources.
• Document medical data and patient communications accurately.
• Assist patients with insurance enrollment through the Maryland Healthy Babies program.
• Track pregnant patients and newborns at Chase Brexton Health Care Glen Burnie and Columbia Centers and follow up on missed appointments.
• Coordinate linkage to internal and external resources to address barriers to care.
• Maintain discrete data in the EMR for tracking and trending.
