Job summary

Aledade PBC is seeking a Practice Transformation Specialist to interface with providers and drive value-based care initiatives across partner practices. You will implement programs, support care transitions, and train staff while ensuring adherence to transformation plans.

Applicants should have 3+ years in medical practice management, strong communication, and familiarity with EHR workflows. A travel component up to 25% is required; Maryland/Delaware market focus.

Responsibilities

• Train office staff and providers on processes and technologies.
• Assess training needs and foster relationships with physicians and healthcare executives.
• Travel up to 25% across Maryland/Delaware market for practice visits.
