Job summary

Aledade PBC is seeking a Practice Transformation Specialist to support South Carolina practices, driving value-based care initiatives and performance improvements. The role interfaces with providers to implement wellness visits, care transitions, and practice management enhancements, aligning with our mission to improve patient care while reducing costs.

Residents of South Carolina are preferred; travel up to 25% within SC may be required as part of practice support.

Qualifications

• Must be based in South Carolina.
• 4–6 years of medical practice management experience or equivalent administrative clinic experience.
• Strong communication, organizational, project management, and team-building skills.
• Experience with EHR systems for clinical/practice management processes.
• Proficient computer skills and willingness to learn new software.

Responsibilities

• Implement products and support ACO initiatives per Market Leadership.
• Assist in wellness visits, transitions of care, and active management to meet Practice Transformation Plans.
• Train office staff and physicians on processes and technologies in coordination with the curriculum.
• Assess ongoing training needs of practices and staff.
• Develop relationships with physicians, providers, and healthcare executives to improve financial and quality metrics.
• Relay practice concerns to management to identify opportunities for improvement.
