Job summary

Aledade PBC is seeking a Practice Transformation Specialist to support our Flex Field Practice Transformation team. You will remotely partner with healthcare providers, onboarding practices and guiding transformation plans to improve patient outcomes in value-based care.

Qualifications

• 4-6 years of significant work experience in medical practice management.
• 3 years of experience with Electronic Health Records (EHR) for clinical/practice management processes.

Responsibilities

• Direct Practice Support across markets, onboarding practices, coaching on technologies, and guiding Practice Transformation Plans (80%).
• ACO Training and Development to train office staff/physicians on processes and technologies, assess training needs, and nurture relationships (10%).
• Customer Success by listening to concerns and relaying information to Managers/Leads to identify improvements (10%).
