Aledade is hiring a Practice Transformation Specialist to act as a frontline champion for partner practices across the US. You will deliver scalable, expert-level support to ensure practices receive consistent, high-quality guidance in value-based care.

The role emphasizes remote work across US time zones, impactful relation-building with providers and leadership, and ongoing support to improve patient outcomes while driving healthcare transformation.

Qualifications

• 4–6 years of significant and relevant work experience in medical practice management.
• 3 years of experience with Electronic Health Records (EHR) for clinical/practice management processes.
• Excellent oral and written communication, organizational and project management skills.
• Familiarity with process mapping and work-flow analysis tools.
• Willingness to learn additional software applications.

Responsibilities

• Direct Practice Support: onboarding practices, coaching on technologies, and leading initiatives to meet Practice Transformation Plans.
• ACO Training and Development: assist in training office staff and providers on processes and technologies.
• Customer Success: listen to practices' concerns and relay information to improve opportunities.
