Job summary

Talentify is seeking a Post-Acute Services Director in Everett, WA to drive growth for our nursing centers by expanding hospital referrals and managing patient transitions.

You will lead relationships with hospital systems, optimize the referral process using CRM, and implement sales and clinical programming strategies to sustain market presence. A background in clinical care and strong leadership are essential to exceed admission targets.

Qualifications

• RN or LPN licensure preferred, but not required.
• Valid driver’s license.
• Five to seven years of recent clinical experience.

Responsibilities

• Lead the patient referral process from review to acceptance with proper handoffs.
• Establish hospital referral source relationships, targeting 5–10 new contacts per month.
• Communicate clinical strengths of centers to align with hospital referral needs.
• Achieve a 60%+ conversion rate for eligible referrals and drive improvements within 90–120 days.
• Develop a market strategy using data analytics to meet admissions and sales goals (≥5% per period).
