Talentify seeks a Post-Acute Services Director to grow referrals and oversee patient transitions across assigned nursing centers. You will manage discharge processes and build enduring relationships with hospital systems to sustain and expand market presence.

The role emphasizes CRM-driven coordination and cross-functional collaboration with clinical teams. Ideal candidates have 5–7 years of clinical experience; RN or LPN licensure is preferred, and a valid driver's license is required.

Qualifications

• RN or LPN licensure (preferred) and license in state of employment.
• Valid driver’s license is required.
• 5–7 years of recent clinical experience.

Responsibilities

• Lead patient referral process from review through admission.
• Cultivate hospital referral sources and 5–10 new contacts per month.
• Communicate clinical strengths of nursing centers to drive referrals.
• Aim for 60%+ conversion of clinically and financially appropriate referrals.
• Develop market strategy plans and meet growth targets.
