Job summary

Genesis Healthcare in Albuquerque seeks a Post-Acute Services Director to drive growth for assigned nursing centers by cultivating hospital relationships, managing transitions, and implementing sales and clinical programming strategies.

You will lead the referral process using CRM tools, build 5-10 new hospital contacts monthly, and ensure smooth handoffs to improve patient outcomes and center performance.

Qualifications

• RN or LPN licensure preferred, if required by state law.
• Valid driver’s license is required.
• Five to seven years of recent clinical experience.

Responsibilities

• Lead the patient referral process from review through admission with smooth clinical handoffs.
• Establish relationships with hospital referral sources and decision-makers.
• Articulate clinical strengths of centers to meet hospitals' referral needs.
• Achieve a 60%+ conversion rate for appropriate referrals and drive improvements quarterly.
• Develop market strategies and exceed admissions and sales goals with a 5%+ rise per period.
