Job summary

Genesis HealthCare, Inc. is seeking a Post-Acute Services Director to lead growth across assigned nursing centers.

You will manage patient referrals, discharge processes, and relationships with hospital systems to ensure smooth transitions and strong market presence. You will cultivate hospital referral sources, document steps in the CRM, and drive strategically focused programming to increase admissions while delivering clinically excellent care.

Qualifications

• RN or LPN licensure preferred but not required by state.
• Valid drivers license.
• Five to seven years of recent clinical experience.

Responsibilities

• Lead the patient referral process from review to admission, ensuring smooth clinical handoffs.
• Develop hospital referral source relationships and target 510 new contacts per month.
• Position nursing centerslincial strengths to meet hospital referral needs and drive referrals.
• Achieve 60%+ conversion for appropriate referrals and improve rates within 900 days.
• Develop market strategy with data analytics to exceed admissions goals (>=5% increase per period).
