Job summary

Tenet Healthcare is seeking a patient-centered, detail-oriented professional to coordinate post-acute referrals under RN/Social Worker supervision. You will arrange post-discharge services, process referrals, verify patient letters for signatures and communicate with patients and families to ensure smooth transitions.

The role requires strong organizational and communication skills, with 2 years of clerical/healthcare experience; nursing assistant, EMT or paramedic certifications are a plus.

Qualifications

• Minimum education: High School diploma or equivalent.
• Preferred: Associate’s or Bachelor’s Degree from an accredited college.
• Minimum experience: 2 years in clerical or healthcare field.
• Strong organizational, verbal/written communication, problem solving and computer skills.
• Preferred: Acute hospital experience.

Responsibilities

• Arrange post-discharge services.
• Create and follow up on electronic referrals using the Tenet Case Management system.
• Review patient letters with patients/families for signatures.
• Provide follow up messages to Medicare patients prior to discharge.
• Communicate with patients, families and care team.
• Complete tasks assigned by Case Manager and Social Work staff.
