Tufts Medicine is seeking an RN Navigator to serve as a liaison between patients, community resources, and providers, supporting a smooth transition from hospital to home with the goal of reducing readmissions and ensuring continuity of care.

The role focuses on coordinating multidisciplinary care, educating patients, and supporting informed decision making, with responsibilities that include post-discharge follow-up and advocacy within the healthcare system.

Qualifications

• Massachusetts RN Licensure is required.
• Minimum of 3 years RN experience.
• Current Basic Life Support (BLS) certification is required.

Responsibilities

• Coordinate multidisciplinary care across the care continuum.
• Provide communication, advocacy, and education in a culturally competent manner.
• Triage new patient referrals to ensure timely access to care.
• Conduct post-discharge follow-up calls and act as patient advocate in navigating care.
