Responsibilities

• Develop and maintain referral relationships with hospitals, physicians, case management teams, and community partners.
• Serve as primary liaison between referral sources and facility clinical/operations teams.
• Identify opportunities to expand strategic accounts and provider relationships.
• Represent the organization at community outreach events, healthcare networking events, and industry meetings.
• Drive skilled nursing facility census growth through targeted business development strategies.
• Monitor referral trends, conversion rates, and admission outcomes.
• Work closely with Clinical Teams to ensure appropriate patient placements and smooth hospital discharge transitions.
• Collaborate with regional leadership on sales strategy and performance improvement.
• Provide leadership with insights regarding growth opportunities and challenges.
