Responsibilities

• Track patient populations to obtain statistical information for program goals.
• Obtain health care information from patients and maintain care management databases.
• Identify patients within care programs using established measures.
• Develop letters and forms to educate and retrieve information from patients.
• Notify enrolled members and track attendance in care management programs.
• Distribute educational materials and support patient education.
• Contact patients by phone and mail to reinforce treatment adherence.
• Generate reports on care management and program performance.
