Job summary
Comprehensive Rehab Consultants (CRC) invites skilled PM&R clinicians to join a physician-owned, rapidly growing team in Middleborough, MA. This full-time role focuses on delivering specialized PM&R services exclusively at designated SNFs or post-acute centers, with strong clinical leadership and collaboration across disciplines.
The ideal candidate holds an NP/PA license, Massachusetts licensure, and US work eligibility, with a passion for geriatric post-acute care and a collaborative approach
Qualifications
- Nurse Practitioner, APRN, or PA license required.
- Massachusetts license required.
- Ability to work in the U.S. is mandatory.
- Post-acute/SNF experience preferred.
- Strong patient care mindset and teamwork.
Responsibilities
- Provide PM&R/Physiatry services as a rehab consultant.
- Represent CRC in your facility with clinical leadership.
- Collaborate with PCP teams for SNF care management.
- Work with PT/OT/SLP to improve patient function and independence.
- Track therapy progress and address barriers to therapy.
- Attend multidisciplinary meetings and bridge therapy with internal medicine.
- Engage in culture-building activities and ongoing education.
