Comprehensive Rehab Consultants seeks a PM&R clinician to provide specialized Physiatry services at a designated SNF/post-acute rehab center. The role emphasizes clinical leadership within a multidisciplinary team to optimize patient independence.

Qualifications

• Experience in post-acute space preferred.
• Ability to work autonomously in SNF/post-acute settings.
• Strong interpersonal skills for facility partners and patients.

Responsibilities

• Provide PM&R/Physiatry services as a rehab consultant.
• Represent CRC in facility with strong clinical leadership.
• Collaborate with PCPs and therapy teams to manage SNF conditions.
• Work with PT/OT/SLP to improve patient function and quality of life.
• Attend multidisciplinary meetings and bridge therapy with internal medicine.
• Support culture-building and ongoing education within the team.
