Comprehensive Rehab Consultants seeks a Part-time PM&R Clinician (NP/PA) to provide specialized PM&R/Physiatry services at designated SNF/post-acute centers in Marlborough and Worcester, MA. You will collaborate with interdisciplinary teams, promote patient independence, and help manage common post-acute conditions.

Qualificações

• Nurse Practitioner/APRN/PA license.
• Massachusetts license.
• Ability to work in the U.S.
• Experience in post-acute space preferred.
• Great attitude and desire to deliver the best patient care.

Responsabilidades

• Provide PM&R/Physiatry services as a rehab consultant.
• Represent CRC in your facility with leadership and collaboration.
• Collaborate with PCP teams for SNF conditions such as CVA and post-op ortho surgeries.
• Work with PT/OT/SLP teams to improve function and quality of life.
• Attend multidisciplinary meetings and bridge therapy and internal medicine.
• Engage in culture building and ongoing education.
